Community Connection of Northeast Oregon

Advocate for and assist senior citizens, children, low-income persons and persons with disabilities in attaining basic human needs and in becoming more self-sufficient.

Community Connections

Community Connections offers services to adults with disabilities at 8 locations throughout Southeastern Massachusetts and Cape Cod.

Through personalized case management, we design individualized programs that build on the strengths of the whole person. Programs are tailored to:
  • Promote self-sufficiency, personal growth and independence
  • Increase access to the community
  • Assist in meeting personal and professional goals
  • Increase social interactions with peers and colleagues
  • Stimulate involvement in civic and community activities
Fulfilling our commitment to coordinate and arrange services that allow our clients to live as independently as possible, our staff:
  • Actively involves clients and their families in planning services and assessing needs
  • Identifies and locates appropriate services and funding sources
  • Advocates on behalf of clients
  • Coordinates the process with clients
Community Council (Dallas AAA)

House of the Dallas Area Agency on Aging and many other supplemental initiatives and evidenced-based interventions designed to support seniors and their caregivers.

Community Crossroads

Provide people in need of long term supports, either through age or disability, with the information, guidance, support and advocacy they need to remain in their chosen homes and live full, independent lives.

Community Entry Services

Provides supports to individuals with developmental disabilities and acquired brain injuries in the State of Wyoming to maximize independence.

Provide legal and advocacy services, training, education, and related activities to persons with disabilities and, in certain issues, to organizations representing their interests, with an emphasis on those most vulnerable and needy.
